New Patient Information and Forms

Fairview Dental Arts aims to make your dental appointment as convenient as possible by offering a wide range of appointment times and by ensuring that our patients never wait. To save even more time, you can download and complete required new patient paperwork prior to your appointment.

 

During your first visit, we will complete a comprehensive cleaning and oral examination. We will screen for decay, gum disease, oral cancer, TMJ, and problems with your bite. We can also address any cosmetic dentistry concerns you may have. In addition to preventive general dentistry and cosmetic dentistry, we also offer dental implants to meet all your oral health needs.

 

To maintain our no-wait policy, we kindly ask that you please give us 72-hours advance notice if you need to change or cancel your appointment.

This is by far the best dental practice I have been to. The moment I walked through the door I was welcomed by a very friendly receptionist with fabulous pearly whites. She was quite thorough with explaining my insurance coverage.
